A simple contract compliance notice containing the necessary information needed under the new GOS conract. You are free to use a different notice or notices to display this information if you wish however this is an approved notice with all  the required information and is the easiest way to ensure compliance.

Edit the document using Word to include the name of the person responible forhandling complaints at your practice

Select the correct form which matches the PCT with whom you contract for GOS

Mobile contractor patient information leaflet

There is also a suggested sample of the mandatory Patient Information Leaflet that all Mobile Contractors providing Domicliary visits must have available. As always you are at liberty to provide your own leaflet but since this one is already approved as containing all the required information, it is well worth a look at first.

Alternatively you may edit this one and print it out. Note it will need a fair bit of editing to include all the mandatory details about your buiness and services!
