This is a Word template form of the GOS 18. It generates a blank GOS 18 form in Word which can be filled in on screen then saved as a Word document and printed out. This removes any issues of legibility of handwriting but retains the familiar GOS 18 format which GPs and hospital staff recognise. Many GPs prefer to receive GOS 18s printed in this way.

The template needs a little editing to include your practice details and  optometrist(s)  name and type of field equipment used. Instructions on editing and using the template can also be downloaded from this page.

The template works on Word 2000, Word XP and Word 2003. It has not been tested on Word 95 or Word 97 but may well work so give it a try
