DIRECT CATARACT REFERRAL SCHEME

This scheme has been in operation for a few years now. All cataract referrals should only be made by an accredited optometrist.

Please be aware that all patients eligible for direct cataract referral in MK should be shown the current "Choice" leaflet, available from the address below. They may then decide to inform you of their choice of where they would like to have their surgery, which you may indicate on the form, or they may decide to contact the support service themselves. In either case, referrals must be sent to:

This is also the number to use if you need to chase a cataract referral.
  
Referrals in MK should never be sent directly to the provider, even when the patient has indicated a choice.
