An audit is being performed as part of an MSc thesis at City University in conjunction with Moorfields Eye Hospital on the new referral form.
After viewing or using the new referral form would you please complete an anonymous, short (approx. 15 mins.) online questionnaire; the results obtained will be analysed, and written up as part of an MSc thesis that is reviewing the use of the new referral form.
