Rotherham Orthorptic Patient Discharge Information 

The following are the 3 type of letters that an Optometrist or Dispensing Optician are likely to see in the course of their routine practice. All 3 copies can view via the single PDF document on the Left titled RGH Orthorptist Letters.

1/    Discharge From Orthoptist Care
2/    Continued Care Requiring Refraction
3/    Incorporating Prism

These should be the only letters that we see coming form the Orthoptists at the hospital. If for some reason any other letter should land in practice, please provide a copy to the LOC for it to be investigated further.

A full eye examination on all of the above patients should be conducted under GOS using the code 5.1 (presented for a test at the request of a medical practitioner).
