Please make the primary reason for referral clear on each referral letter.

NHS Forth Valley operate low vision assessment clinics through the HES.

Please refer any patients who require a LVA assessment to the Forth Valley Sensory Centre at Camelon, Falkirk,FK1 4DD  either by direct referral or via the local hospital eye department.
