Fast track referrals for patients with a Dorset GP suspected of having wet AMD should be faxed to either of the following:

Practitioners can either use the attached rapid access referral form or a referral letter or form of your choosing as long as it has the relevant patient contact details & clinical information.
  
This service is for patients who have recent symptoms of visual loss, spontaneously reported distortion or blurred central vision & VA in that eye of between 6/9 and 6/96
